2001 brake problem

2001 brake problem
About 5% of the time, when I brake, I have to put the pedal almost all the way to the floor. If I pump the pedal a couple times it gets firm again. It's enough to really scare me when exiting the freeway or stopping at an intersection. Has anyone had this problem or know what it might be? I'd like to fix it before something really bad happens.
I checked all the pads and rotors, plently of life left.
