No matter what , I got hot air from the REAR A/C unit
I opened up the side of the van, and found the REAR MODE DOOR was detached from the Vacuum actuator, thus needs to be replaced.
I am wondering the following:
1) Part number for rear mode door
2) Part number fro vacuum actuator
3) Also the heat is ALWAYS ON, is there a control valve on this? or not?
(I hear some vans do have this, some just always run hot water to the heat exchanger.. always)

The hot coolant to both heater cores is always on--temperature of blowing air is controlled by the blend doors which open enough to satisfy the temperature control **** settings. IF your van is equipped with the rear control it will act mostly independent of the front settings. I have the complete electrical schematic for the heater-A/C system blowers if you need them.
As an aside there are ways to completely shut off hot coolant flow to the heater cores, its been described as the "cold air option" in different forums on FTE. Its a bit involved so depending on your skill level and desire its true value really is subjective. I've done my own version which only duplicates the Max A/C setting. In order to accomplish a true flow stop I'd have to add a few electric or vacuum control switches---not sure its really worth it though.

We just ran into the same issue with our '96 E350. Same thing, the actuator was somehow broken off of the motor...not sure exactly. We decided that we can live without rear heat (this is a summertime use vehicle). So, we just wired it into an A/C all the time mode for now. If we end up needing heat at a later date, we can always go back in and fix it. In the meantime, if anyone has any hints as to what broke or what to fix, please post.
HotAir what sort of E350 do you have? Straight cargo-only or Club Wagon with full interior?
The actuator is an easy swap but access may be hindered by your van's configuration---it seems you've at least found the rear heater/AC unit. Maybe a few photos will help:
Its possible the diverter door has seized in place and dislodged the actuator. Your first step is to get a view of your unit and maybe post a few photos too.
JWA, our's is a Clubwagon. I wasn't the one working on it, so I can't say for sure what was wrong. The guy working on it said it seemed like the actuator was disconnected from the door. I had him wire the door into permenant A/C mode so that we have A/C for the summer. This van gets idled for the winter, so it should be fine like this for the foreseeable future. So, unless we have some other reason to tear it apart in the future (knock on wood that we don't), I'll probably just leave it like this.
In this configuration, we have A/C when the compressor is on and recirculated air when the compressor is off. If we need some heat, we can run the front on heat, and recirculate the rear air. That should be sufficient for what we do.
I had the same problem with a 93 e150.
Check if you have a hot water valve on the underbody driver side between the front and rear wheels. It's vacuum operated , mine needed a vacuum line and that fixed it.

My valve was a coolant leakier a bit back too! Easy replacement.
