Sometimes when I brake, my foot also touches the accelerator pedal. If my foot is positioned just a little too far right on the brake pedal and if I brake very hard at all then it also pushes the go pedal.
On the way to the post office this morning someone decided to stop for a yellow at the last second. I braked but my truck kept going. The harder I pushed, the harder it accelerated, so much so that it started spinning the tires. I couldn't feel that I was pressing on the accelerator but quickly realized that I would have to bring my left foot into action on the brake and removed my right foot completely. (Consider that I was just a foot or so off her bumper by this time so I couldn't just let up on the brake)

I agree. Call the DMV and tell them you need a driver's test. The space between the pedals is huge and I cannot imagine not knowing you're pressing the gas and brake at the same time. Seriously.
The distance between the brake and accelerator pedals in these pickups is not sufficient.
In my Chevrolet, I could fit my whole foot (7-1/2 E) between the pedals (manual on both, so the brake is not the wide brake).
In my '08 Ford, I can mash both pedals down with ease. I have it easy because my clutch is in, and my hand is on the shifter... automatics might have a bit more trouble.
Disconnecting the accelerator while braking is a horrible idea. The Toyota/Lexus problem was with disconnecting the cruise control.
As far as not knowing, the drive-by-wire accelerator they put in these days takes very little effort to press. Much less than the brake, so if you are putting out braking effort to the pedal, you'll never even fell the accelerator... you just hear the engine wind up.

Simply not the case, I never once had it happen with my 04 or 06 f350 and three times now with the 08. Something changed there is no doubt in my mind. Just have to more mindful and move my right foot over to the left more is all.

Toyotas killed people because gas pedal got stuck by the carpet and with the absence of SAFETY feature of disconnecting the accelerator while pushing the brake the drivers were not able to stop the car. It is a bit more complicated, but bottom line is that other brands do have the right program and never had the problem the Toyota did.
Superduty with its big pedal not likely will have carpet holding the gas pedal, but we did have a recall for wrong wiring on CC didn't we?

If you think the space is too tight, I wouldn't try driving a manual-trans performance car. Or any car until you find the problem...
Why could they not just put the dang thing in neutral? I have taught both my teenage boys this if they were to ever have a throttle cable hang up or something. I just can not understand why these toyota drivers rode the thing out into whatever they hit. Either stick or auto you can still put the thing in neutral right?
Bear in mind that digital vehicles do what factory programmers told them to do, not what the driver wants.
My main truck is having manual transmission, but I am planning to get on my new purchase with automatic and see how it works.
Those things sound scary.
