How will I know?

I am seeing more and more threads on valve lifter failure and how devesataing / costly it is once it happens. How can one know if it is happening or working towards that way? I am new to the 6.0 and still learning, but trying to gather as much info as I can to avoid excessive expense. What is the main cause of the failure? At what mileage would a failure of this type become a reality, on an engine that gets regular maintenance? Would I see signs in the drain oil, filter or drain plug? I know they may seem basic questions / knowledge for the dealership mechanics, but Im coming from more of a heavy equipment background and not the 6.0.
My vehicle is an 06 with 121K, regular maintenance, oil and EGR coolers replaced at 115K and upgraded exhaust. Only problem I have right now is stiction on cold days.

If your running oil samples...might notice a spike in nickle (I think) shortly before one starts to go; but you wouldn't have allot of warning. Most guys that are seeing the lifters go are well above 250k miles...so I'm of the opinion they just finally wore out. With 250k miles, that's a fair amount of mileage on these chassis if you ask me. This is all assuming your doing your PM with OEM spec filters & fluids on time.

Thanks I will certainly be reading what everyone has to say. Right now I keep the fuel filters to every other oil change and oil changes to 4K and the drain oil is always just starting to darken. I know it may seem a little premature, but I would rather spend the extra $150.00 yearly in preventative measure than the thousands later.
