They are machined and reamed to size. Just like anything else, it takes experience and skill to get the clearance correct. Too loose, it burns oil, too tight and you get this problem. It's got to be just right.

A good head assembled can tell when they need a bit more reaming just by how the valves feel as they put them in, but that is rapidly becoming a lost art......

As I said in the beginning of this post. The truck didn't run, so I pulled the heads and had them re done. Apparently, the machine shop didn't do something right with the valve guides, and they were too tight. Sounds weird, but nothing else seems any more normal.
