Missing/Surging at cruising speeds

Missing/Surging at cruising speeds
When the truck starts up white smoke rolls out. It runs great in every gear but 5th at low throttle at cruising speeds. I have changed fuel, Oil, and Transmission Filters and Fluid. I tested the Ficm and voltage is normal. I changed #6 Injector had ford update the computer to the latest updates. Its not chipped. there is one code that pops up from time to time P0700. Can anybody tell me how to fix this before it causes more damage. Thanks in advance.
