I'm setting all of them on a buzz test. Only thing I can think of is the IDM is about gone. It runs, but badly. Smokes, misses, looses power, it's come to a halt more than once, but eventually fires back up and sometimes will run great (but not for long).
NA11 DIAGNOSTIC TROUBLE CODES (DTCs) P1271-P1278
DTCs P1271-P1278 indicate a low side-to-high side open in the circuit between the IDM and the injector. The following table lists the circuits to inspect while performing these pinpoint tests, based on the DTC retrieved during the KOEO On-Demand or Injector Electrical Self Tests.
Note: If all of the DTCs associated with an entire bank are retrieved, the high side circuit is most likely open.
I'd check the pins and connectors at the Valve Cover gasket both inside and out and work toward the 42 pin connector and on back to the IDM before condemning the IDM.
I saw this in the other thread, but I'm setting the code on every cylinder (and it still runs). The plug-ins look good under the covers and we cleaned up the connector at the IDM. This is actually a 98 E350 so it's a little harder to get to the wiring. It had all the injectors replaced around 180k and it has just over 200k now. It's been running bad for a while now but it always manages to limp home.
Have had 3 idm failures. Most often the truck just died and wouldn't restart but one time it would restart and run but very rough and underpowered. It didn't really go away and come back again as you are describing.
I'll admit we are grasping at whatever we can on this one. We've done a balloon test looking for a bad injector and can't find a one. We replaced the electric fuel pump a couple months ago and thought we'd found it, but that just got it back home.
It's running pretty consistantly rough right now. Maybe something else will present itself...
We put in a new IDM tonight and it started right up smoother than ever! Ran great, but it's still smoking quite a bit. I'm guessing it's from all the unburnt fuel in the exhaust system. Hopefully it'll burn out on the first long pull. I'll let you know if it's cured in a few days.
Well, I guess it still has a problem. It made it about 20 miles and started running rough again. Getting all kinds of codes but all having to do with injector wiring. I just wish I could find something wrong with it...
Of course now that it's back in the shop it's running smooth again. This thing is driving us all to drink. I notice the electric pump sounds like its grinding coffee most of the time, I wonder if it's pulling air somewhere?

Could be a restriction in the tank. What is the ICP pressure and duty cycle at idle? Do you have any way to watch fuel pressure while under load?
I think we'll just drop the tank and check it out. We did try and run it with a hose into a can of fuel last week and I thought the pump at least sounded better. I'll let you know what we find in there.
Wow, what a crazy setup inside that fuel tank! Being a van it's just a big square tank with a fuel catch bowl in the middle for the suction pipe to sit in. We pulled the strainer out and found quite a few issues. First thing we saw was the anti-siphon cap knocked in the bottom where it could be pulled under the strainer. There was also a big white plastic piece that I wasn't sure about... It turned out to have 2 very fine nylon screens in it, that were both plugged SOLID. After a clean up and re-install it is running pretty good now. It's already made a 40 mile round trip as a test run without any issues.
As long as the fuel starvation didn't hurt the new fuel pump and (recent) injectors.........
