If you run out of oil it will jerk, like a gasser missing. Hesitiating is something else, I believe. When I first got my truck, pulling a load at full throttle with stock injectors and HPOP it would jerk. Replaced with a SD HPOP and the jerking went away.
I've never had a jerking issue.
It is more of a take off and go, and you know there is more there, but the engine just isn't giving it its all.
It's like an electric motor that needs the brushes cleaned.
I've driven mine for over a year with the 15 in it and had no problems unless I was doing hole shots. Never ran outa oil hauling anything.
I agree that finding out the problem with the jerking would be at the top of my list before doing any mods.
What kind of oil are you running? Any oil additives?
Are you consistently adding any fuel additive/injector cleaner to the diesel? Lucas diesel fuel additive is OK, but Power Service is just as readily available and better in my opinion.
Have you changed the fuel filter recently? If so, what did it look like?

Lucas fuel treatment is super bad dude!
dont ever use it.
They tested it, and it caused negative lubricity with the diesel fuel!!!!!!!!!!

Right now I'm running Motorcraft 15W40 (the "ford diesel recomended" stuff). No oil addatives. No, not adding lucas or other "cleaners" concistantly, I've only added it 3 times in the 2 years I've had it... I just changed the fuel filter yesterday, and I'm not sure what it looked like new, but it was jet black... the fuel in the bowl looked clean, and I haven't driven it since I swapped the filter, so I'm not sure if it's better now...
136,000 miles is quite low for a 17 year old truck. Although it is low in miles, it has a lot of time on the clock, meaning it is a 17 year old truck. The o-rings are possibly leaking on the injectors, IPR o-rings, weak fuel pump, injectors partially plugged up, several possibilities. Or a combination of things causing the rough running.
Bio is an excellent cleaner and lubricant for the fuel system. However, it does such a good job at cleaning that it can cause fuel filters to plug up prematurely while it's cleaning the fuel tank and lines. Once it's all clean, you should expect to get a normal life out of your fuel filters.
