E-brake

How do you tighten up the e-brake cable??? They used to have threaded rod on the older ones but there is nothing like that now. This has been going on for awhile, I know it's not the brake shoes or and adjustment. The e-brake goes all the way to floor and even on a slight incline the truck will roll (slowly) the brakes work good. Is the cable just stretched??? is this a common occurrence ???
