I seen rods tossed out(almost took my foot off) but not a piston ejected out like that. I have heard of that happening before on marine engines when the props cavitate and engine over speeds.
i have had that happen. we have an international 966 tractor, and it spun a bearing, and got a little hot. it blew chunks of block and piston over 25 feet. its a good thing no one was standing next to it.
Somewhere on youtube(can't find it now) I seen a video of a tractor puller blowing a engine cylinder bank off the crank bed and runs over it. Just con rods and pistons and crank was sitting there.
On the aircraft carrier I was on we had a main feed pump to the boilers overspeed, throwing it's steam turbine parts up 7 decks of steel, embedding into the ceiling of the 8th deck, missing a main power switchboard and countless people. Yep, power can be a beotch if not controlled...
