codes on 2000 7.3

codes on 2000 7.3
have a 2000 7.3 that is very hard on fuel and has codes coming up on it changed the exhaust back preassure sensor and tube still didnt help
when climbing hills or under a load it makes a wierd hissing sound making it sound that its starving for air the codes coming up are 1211 witch is icp and 1249 west gate perfomance and while doing this check engine light comes on and off and if you dont let off a bit it will come on and stay on for a little while go out and away truck goes
i took the icp sensor that goes into the top of engine out to look at it and cleaned it and put it back in with wires unhooked i started truck fired up ran fine check engine light on read codes and it came up as 1280 so its not this sensor
