I would go with a second alt. If u buy the pully that goes on the crank pully from a snow plow company and get a used alt. And place the.simaler style pully on the alt u can get more juice. The old 7.3 idis ran a v shape belt on thier altintator it wasnt a serpintine belt so im not sure if they ran for volts but thats the style I would use

Actually it was a friend's truck and for the cost of a few diodes and some aluminum channel to make a mount/heat sink why the heck not.

Of course you have to find one with the same engine that you have so the brackets you leech from it simply bolt right on.
Remember though that ambulances are generally aftermarket produced as Ford sells ambulance companies incomplete vehicles.
But adding a second alternator is just the matter of deciding where it fits, making a bracket, and making sure you have enough pulley wrap and enough tension.

I'm pretty sure it will have to be a custom job for me anyways, cause I got all sorts of idlers on my engine that normally don't belong there. And I don't think I've seen an IDI-powered ambulance with two alternators, but maybe I just didn't look hard enough...

Some people move their one alternator down there to clean up the top of the engine, so I suppose you can easily install a second one there while retaining your factory one where it normally lives.

Well that's a neat trick to fit two batteries in the truck without going diesel-style and moving all sorts of things on the driver-side fender to fit a factory second-battery setup there. Of course, those of us who have diesels, we can use that tray to run three batteries instead of just two, cold starts will like that for sure, lol.

I've seen a few IDI ambulances here and there but I don't see that many ambulances in junkyards in general.
Making brackets isn't that difficult though. The "trick" is to think three-dimensionally and waste a lot of cardboard before wasting steel or aluminum.
