I'm looking for a good, comprehensive list of preventative maintenance that should be done around 200,000. I'm at about 220,000 right now so I'm running a little behind. She's been running great but I want to stay on top of things. Let's hear it.

Funny, I was going to start a thread with a similar subject. Problems I've had on my trucks that could have been prevented: bad battery (died suddenly in the middle of the left turn lane. In rush hour ), blown rad hose (middle of nowhere). Rusted out brake line (could have been much worse).
I'd also think about the timing chain, if that hasn't been done yet. Check your leaf springs for cracks.
Change all the fluids. Oil, coolant, tranny, diff(s), ATF in the powersteering, brake fluid, fuel filter, check the shocks, change serpentine belt, inspect or change radiator and heater hoses, clean and check battery cables and grounds.
I think that's about it. 200,000 miles was so long ago I don't remember what all I did, but it seems like I did the above stuff at 300k.
