Thought I'd post here as well as the older 7.3 as its engine related
Quick summary, sometimes a stutter at idle just after loaded, meaning I push the clutch after idling around a parking lot (stutter meaning the cab shakes, and it sounds like its gonna die, RPM's to around 400-500). Also, if I try to rev the engine to 3k in neutral it hesitates around 2500, then goes to 3k. The first time I try, I get a huge cloud of white smoke. The next time I try, still hesitates but no smoke.
So I changed the injector o-rings, seems like maybe the stutter is less frequent, but still have the hesitation and smoke when rev'ing the first time. When stutter happens, it is bad.
I have an IPR on the way...but now that I have the smoke again I don't think that's it...
Your best bet is to find someone who can do a Buzz test and listen for a weak one. Sounds like what injector failure or unver valve cover harness problem. If you can't get to AE for a buzz test, buy a infared temp gun and shoot the manifold at each exhaust port outlet. I usually bring it with me, run down the road and get the egt's up, then pull over and do the test quick. Very easy to tell which ones are not working (they will be the cooler ones)
Most diesel shops have a way to buzz test your injectors. I recently replaced a bad glow plug and knocked an injector plug in the process (bummer). Had to take the valve cover off a second time to fix the issue. My truck had similar problems at idle/rev. I'd get them tested first, you'll probably figure it out. Good luck man.
