Hi Mark and welcome to the forum
What size engine in your truck? What arm do you mean?

I havent had one of those apart in a long time but I dont think you can replace just the upper tabs. Maybe find a good used one at a wreckers?
The synchronzer has to be timed correctly with a special tool when it is installed or it can cause a rough run.

Hey, That happend to me also. I have a 97 4.2 . Canshaft Position Sensor went up - the little magnet was missing, and you could put your finger right through the hole where the magnet was suppose to be.
I seen the syncronizer shaft and the metal tabs were bent also like you describe. The mechanic told me that the shaft could also starve your engine from getting *oil. Im not sure if that's true though. He told me briefly that this is some how connected to the oil pump? It costed about 300 to get it all done.

Earl
