The two bad looking one look like bad O-rings on the bottom. There by letting fuel down to the brass ring. Could clean them up and reinstall and see what happens, or send them out and have them cleaned and tested. I go the first route first, all with new O-rings and washers.

Also will be cleaning the cups and inspecting those.

I have my injectors out for new I rings right now, they all look about like that, like the o rings were leaking. The rings I took off looked a lot different than the Alliant rings I'm putting back on, so I'm hoping to chalk this up to Chinese parts store o rings.

Any of the reputable injector builders can flow test them for you
Whomever you might choose down the road would be who I would go to. The test includes any minor repairs that can be made and new o-rings, also if there is an issue Ryan or crew will let you know and give you guidance as to what is needed.
Number 4 is the one that really caught my attention - no copper washer and that black is combustion gasses reaching up into the cup area. Did the injector in the fourth picture go with the third cup picture? This can cause the following:
Planet-splitting cackle.
Bucking at light acceleration.
Cup damage (look real close at that one).
An EPA official to visit you on cool/cold mornings.
An "Is that a miss?" look on your face as you drive.
I think you'll hear a "Tink".
Fully/partially plugged nozzle(s)
Sooty fuel, as well as oily fuel from the bad O-rings.
I urge you to visit the recommendations for oil quality and change frequency... that oil looks rough.
Before you put everything back, do a pressure test on your coolant and observe your cups. You may hate me for this one, but this is the hour. That blackened cup might not seal properly with the new fuel O-ring, the cup may need a little polishing with a Dremel - avoid aggressive bits that can score the sides. Block the oil/fuel rails before taking on such a task.
I'm with Rich here. Knowing just how critical the cup is to the health of these things, and seeing that picture. I'd be, "Oh crap!"..."Clay, I need some cups and a send the tool with."

Thanks Tugly, was hoping you'd chime in here......#4 does have the copper on it, just can't see it from bad pic. The 1st inj pic goes with the 3rd cup pic. I learned how to pressurize coolant system (via YouTube last night) and test the cups, I'm pretty sure at least 2 of them are bad.
As for the oil change intervals, driver was told 5,000 miles and he has let it get to 6,500. The oil in the pic was just changed out 16 miles ago! We didn't change the filter, just drained the oil pan after replacing the glow plugs because we used a little brake cleaner to clean things up and refilled with fresh oil, had a feeling we weren't done yet.
The sludge on the first one has a green tint to it and looks to me like coolant that has been heated,dehydrated, baked? Along with the black nozzle makes me think the cup is done but I will clean them up and pressurize. I don't mind whatever work may need to be done at this point as I'm already committed.
I have already filled my shopping cart @ RiffRaff with cup tool kit, cups and injectors but didn't execute last night. Wanted to get opinions on replacing all cups and injectors while I have it opened since there's 211,000 on it. Don't want to go through all this and end up needing new injectors anyway.
I'm in the dark with getting them tested/ cleaned with so many miles on them they will probably tell me they need to be rebuilt and I'm thinking might as well replace as opposed to rebuild?
Nothing in the fuel bowl, he ran some DK through it not that long ago. Just stopped by HF, checked mailbox first for coupons, and picked up the coolant system test kit.
Also, they had their diesel comp kit on sale for $140 but didn't get it.
I will post results once I get the cups cleaned and tested.
Prepping the truck and getting the tools for a cup job is the hard part. Once you're that far, I say do it all... and I back that up with my actions. Search for "Stinky's Stick-up" and you'll see how a real shlameal steps up.
