It takes 10 minutes or less to remove the inner plastic fender. It's a new world that opens up to that area. That with the tire off and doing as SHO 89 said makes it a lot easier. As mentioned...ONLY USE 6 pt....NOT 12 pt. Tap gently on the socket and extension before you try to turn it. Good Luck!!
It can all be done from underneath. The top is tricky just have to feel your way. I had to change the starter on a 97 expy 5.4 I use to have in Meijer parking lot in the dead of winter.
Like spotty said removing the inner fender.....you can see everything. If your not in a hurry that's your best bet.
Be careful tightening the stater bolts that thread into the aluminum tranny. One of mine (top) was stripped, I had to drill out and tap it one size larger and install a new SS bolt. PITA.

Ha! I had to change one on my old 97 in the parking lot of a lake resort down in Arkansas two summers ago. 105° temp on a blacktop parking lot - that was sure fun!
