rough start no high idle

rough start no high idle
Hi hope there is some help out there
when I start my truck the engine is really rough and will not idle up like it used to for warm up.
Had it in for service and after finally getting the computer port working (another issue, ground problem) showed codes for all injectors. Only had 21 volts off of ficm. Was told to run a good injector cleaner through and let it sit overnight and that might free up the injectors. Did that and rechecked voltage and we started getting 48 volts which is great. Let it sit overnight and tried a cold start again the next day and it started poorly again and upon checking voltage again we got 21 volts again.
I am now at a loss don't know where to go from here. I have heard there is a product called hot shots that is supposed to fix this problem. Anyone tried that or am I following ghosts here and maybe there is a bigger problem.
