Are you refering to the thing sticking out of the door or the whole assembly? The piece sticking out of the door through the skin appears to be like the original Ford design. The rest attached to it is not. Although that is a pretty intersting idea to get a better look at things behind you.
That set up looks almost identical to the setup on my 1964 F-250 I had in the late 1980's. My 64 had an adjustment that allowed the mirror to telescope out. I always kept it in a lot shorter than what that truck has, but it looks like someone replaced the stock wide mirror setup.
I built the same type deal with a shorter shaft for my current 66. I hate small mirrors, and so this was better than the little round mirrors that many of the F-100's had through the late 50's through 66.
