Just to make it easier on everyone else, I am going to list everything that I learned from multiple threads on multiple sites. It took me hours to find all of the pertinant info.

Sensors:

It seems that if your back up sensor is not working, ie, the OFF button stays lit constantly, the most common cause is the sensors. If you can afford it, go to Ford, pay for a scan, and replace the sensor that they tell you is bad. It's not a bad idea to check all the plugs and wiring of each sensor first.
If you want to self diagnose, put parking brake on, turn ignition on (DO NOT START) and put in reverse. Check each sensor by touching or listening carefully. You should hear clicking on each one. If any don't click, check connections. If ok, probably a bad sensor. (thanks for this advice by the way, big time saver!)

Module:
On 2005 F250 (probably all the trucks) is found on the PASSENGER side foot well, WAY up in the dashboard behind the glove box. You have to remove the plastic cover down by your feet, and look WAY up the back of the glove box. I had NO access through the glove box, as the AC condensor was in the way. The module is obvious as it has a small speaker looking thing on it. It is held in by 2 plastic clips which need a clip remover and some time to remove. Once removed, undo the two plugs and remove.
On Excursions, it is in the rear side panel.

Most likely, your module is fine! Again, scan by Ford if you can, but if not, look at the speaker. Mine was cruddy and had built up an accumulation of magnetic gravel that seemed to be gumming it up. It just removed it, cleaned it, and tested it with a 9V battery. After messing with it, it worked! They are really cheesy, and seem to break a lot. The modules are about $200. The speaker is only $20-25 and more likely to be broken. Here are the part numbers for the module and the speaker. Hopefully this saves some of you some hard earned money and time!

Good luck!

This is great, thank you very much! Side note - Tousley is great! He's a sponsor (or at least an active member) on svtperformance.com and he offers parts there for 10% over cost as well! I was VERY happy to see that he could not only serve me with my Cobra, but with my Excursion now as well!

Speaker fixed mine.

Verified speaker was bad. If you put a multimeter between the two contacts you should get continuity. If not, then the speaker is bad.

Here was my simple and cheap fix;

My wife had one of those birthday cards that plays anoying sounds. I took the old speaker from the X and removed the wire connectors with a soldering iron and then solderd them onto the birthday card speakers. Hooked it all back up and now my beeper works.

Darn, I wanted to hear "Happy Birthday to you" everytime I put the Ex in reverse.

She actually did ask that question and I had to explain to her how the electronics work in those things. I seriously thought it would be funny to try and hook up the wires from the truck to the circuit board. The card she gave me had some annoying opera singer.

I apologize for bringing up an old thread, but what online store you guys use to buy lower priced parts?

My local dealer wants $40 for the sensor speaker. Someone said here that they only paid $20 for it. Thanks.

Crew Cab Spekaer Location 2005

I have a 2005 F250 Crew Cab. Where would I find the speaker for the back-up alarm on it? I see no holes in the trim panels, other than the C-pillar trim panel for the radio speaker.

Doh!

Sorry. I misunderstood. The speakers is attached to the module. It just clips on. If you find the module, you have found the speaker. Its way up in the corner. You have to dig for it, but its up there.

yikes! so, on older units it was in the back, by itself? On a 2005, it's on the module?
