I have a 96 Ford Explorer 5 speed tranny. What is the common reason for it to slip out of gear? I have put in a new complete clutch assembly already and changed the fluid.

Welcome to the site. You have the Mazda built "M5OD" trans in there, in the flavor sized to the 4.0. Similar to the F150 and Thunderbird units. You may have shift fork issues. This recent thread went through this, and has links to other threads and sites with some info.

Basically, the floor panel comes up, the top cover comes off, the old parts out, the new parts in.
