Injector failure opinions

I have a 2003 6.0 with 160,000 miles. I replaced the turbo at 90,000, egr delete at 125,000 along with oil cooler replacement and coolant filter added. I've been using Rotella Syn every 5000 miles and recently replaced the fuel filters - always get my filters from ford. I drive around 4000 miles a month and my truck has always ran great until the last few weeks. It quit on me about four weeks ago, towed to dealer and they replaced all four injectors on the right bank. The service writer commented that it was unusual to have that happen but could get no further info as the mechanic was gone and I was picking up the truck after hours. Anyway the truck ran great for a couple weeks, then a little rough , then died again. The shop its in now is replacing the fuel pump and #6 injector. My question is would it be considered "normal" to have such a domino effect with the fuel system considering mileage or could there be something else going on?

Low fuel pressure is a culprit. Did they test fuel pressure?

Gearloose is right on here. Low pressure kills the injectors in a hurry. Stock fuel pressure regulator spring has been proven to weaken and lower fuel pressure. You need a FP gauge

FICM maybe or throwing parts at it? I'm always skeptical about multiple injector failures that seem to happen at once. I suppose that could also be an indication of a fuel problem too. How did your last set of filters look and did they run a fuel pump pressure test on your pump?

The fuel pressure was checked when the first four injectors were replaced, also I was told the FICM was checked , both were said to be ok. Makes me wonder a little now that Im replacing the pump and another injector. I know the 6.0 fuel systems are finicky and I run the truck hard. I will be adding a fp guage after this go around though. Also there is a little more to the story here. Unfortunately, there is sometimes dissention between crews of different companies in my trade. We had five 6.0's in the field during this particular time, two were at parked at a motel where no others contractors were staying and three were at another motel where there were other contractors staying. All three of these trucks broke down within days of one another and all three had injector issues. Fuel analysis was done on one but showed nothing wrong.All five trucks were fueled at the same time at the same stations too. Needless to say I suspect some foul play although Im really hoping not.

Oh and the filters were said to be good too. Only about 3000 miles on them by this time
