Fuel Mileage.

I have a 2002 F250 with stock 265 Tires on it and get between 16 and 20 mpg, depending on load, on the highway. My brother has a 2002 F350 with a 6" lift and 35" BFG's and only gets 13 mpg on the highway. He has cut the muffler out of his exhaust and that gave him about 2 extra mpg.

Fuel Mileage.

Your brother's mileage should have dropped some with the big lift and taller tires. The lift puts the truck up high enough to make it less aerodynamic, and the taller tires are heavier, and have increased contact area with the road, which would increase the rolling resisitance. Both of those are givens. What also might be happening, is that the taller tires have made the speedometer/odometer inaccurate, so the mileage could be better than you think. After you've taken that inaccuracy into account, it might not be as bad as you think.
The truck probably looks real good though, eh?
