Just a guess here:
Being a '96, it should have OBD2, which means it has front 02 sensor/s for the correct a/f ratio, and rear 02 sensor/s to keep tabs on how well the cat is working. Sounds to me like the cat is old and needs to be replaced.

That "bank 1" makes me wonder.I also have a '96 and would want to know what that means,could it be the o2 in the front manifold? The term bank,I've always heard,applied to v8,v6,ect. for one side or the other,just curious and wouldn't want you to set it all up,then still have the problem.May be worth checking into.
