I have 2001 7.3 liter turbo diesel E-350. I have replaced the vacuum pump 4 times over the last four years. What causes the pump to fail?
pump starts to make a lot of noise. Lose pressure on brake pedal. The first press on the pedal has some pressure. The second has almost none. I have had it replaced at two different dealers hips and two different independent shops.

Diaphragm in them usually cracks, but I haven't seen them fail in that short of a time period. Seems odd. How's the vacuum brake booster diaphragm? Does it hold vacuum after you turn of the engine? Did you still have a good pedal for a few stops when the pump was good?
Only thing that comes to miind is a leaking brake master cylinder. If fluid gets in the booster from the rear of the M/C, it could get drawn into the vacuum pump and ruin the diapragm. Just a thought. You could unbolt the master cylinder from the vacuum booster and check this. Has the brake M/C been replaced before?
