Fan issue and tensioner

'95 F150 - 5.0L w/ AC -
Can anyone run through how the fan should be working. At what speed/RPM/engine temp does the clutch disengage? Mine seems to be having oddly. With engine up to temp and idling, the fan seems to be free-spinning, ie not under power.

Also, the top-most idler pulley/tensioner (the one attached to the alternator) seems to be making sounds. Can it be greased or quieted? I know somethings up because I've been getting that annoying 'squeeeel' when accelerating, indicating a slipping belt right?
