2002 Powerstroke
UVCH connections - there is a wire connector going to each of your valve covers. Pull the valve covers off and make sure the connection on both sides is fully connected and not coming loose. The inside connection is known to wiggle loose and cause problems. This isn't likely your problem, but it wouldn't hurt to check out. While you have the valve covers off you should also re-torque the injector hold downs and rocker pedistal bolts since they are right there... I can't recall the torque specs right now but I'll try to find them.
CPS - this is the most likely cause of your problem. Has your truck been in for the cam sensor recall yet? Even if it has you might still have a bad CPS. The CPS is what sends the electronic signal to fire the injectors, its bolted to the front of the motor on the passenger side just above the harmonic balancer. They have quite a reputation for failing, even the new gray sensor issued for the recall. This is where I would start. Its easy to swap out, you need a 10mm deep well 1/4" drive socket and a screwdriver or pry bar to help pop the sensor out.
You could also have a wire that is rubbed through somewhere causing the problem, but right now I'm betting CPS. Start there and let us know if replacing it solves the problem.
