It looks like a water meter (reader) wrench. The "socket" ends are 5 points to loosen the meter lid and the "driver" side is scratch the dirt away from the lid. Ford makes water meters and vaults (pits).

Nah, that's the Rotunda tool for adjusting your Kanooner valve. The old ones had the large nut, the newer modern ones were smaller and use the smaller end.
