When I still had mine on, I couldn't run with the side windows and sliding rear window open as that would pull a lot of exhaust fumes straight into the cab.
If doing one or the other, it was never a problem though.
I haven't had any problems with fumes/smoke. I like to run with all my windows open so I can hear the turbo whistle. My stacks are a little taller than most however so that may play a part.
