need help with C-6

I have a 77 f-150 400/C6. When the tranny is cooled the whole things wroks fine but when it has been runing for 2-3 min it will jerk when put in reverse and after about 15-20 min it has an exagerated delay from 2nd to 3rd gear. Does it need a rebuil or could something else be wrong?
