can't find info on operation of fuel heater in the manual. does it kick on/off with glow plugs, or does it come on and stay on?

The fuel heater is internally regulated.
If you suspect a problem with the fuel heater just unplug it. It's really not necessary unless you live in the arctic.

Believe fuel heater is on all the time (run or start) as it connects directly to fuse #22, no relay.
