I have been banging my head against the wall for a few weeks over a service engine light that gave a "fuel system lean" code and an 99 Expy 5.4 that was idling very rough. Bound and determined to fix it my self I began to look at my options. Maybe a vac. leak. Well I found one right off the bat in a T fitting just above the PCV valve. I thought my problem was fixed. WRONG! After several more hours of serching a few days later I found another leak. Guys if you ever have this problem check over the back of the intake. There is a vac. line about as big as your thumb that rubs on the top corner of the block or something. You cant see it but if you can feel you can find it. I hope this helps someone out and they don't have to go through what I did.
