E350 starting problems

It cranks strong for way longer than it should and then kind of "lopes" to a start. Only does this after it has sat for a while but no real consistancy at all. Other times it will fire right up. I replaced the IAC and it didnt help. Any Ideas?

If the IAC is a good one, then let's rule it out for the time being.

What year model and miles, etc?

(1) Could be the rubber elbow attached to the throttle body - vacuum leak
(2) Do you have humid weather? Could be the MAF sensor covered in condensation.
