I pulled my motor out to install the studs. I have a lot of wires etc running into my cab and didn't want to cut and reconnect all my accessory wires if I had to pull the cab off. Pulling the motor out was not that bad. If I had to do it again I can probably get the motor out under 5hrs easy. I would not advise trying to do a head job with the motor in the truck or not pulling the cab off. You have to torque the studs to 245lbs and requires a lot of leverage and space to get a torque wrench in there with a long enough handle. I recommend that you get your heads resurfaced and magnafluxed. Both my heads were cracked. I bought 3 different sets of used heads, all 3 sets were cracked. I installed a EGR delete kit in it as well, runs great now and lower EGT's. This is the way to do it right.
