91 F-250 4x4 Trans. won't go in reverse

I've got a 91 F-250 351cid 4x4 with a 5 speed manuel transmission. I had to remove the shifter in order to get the transmission out to change the clutch and clutch master cylinder. While out I removed the cover plate that the shifter sits on to look inside the transmission. There is a small metal plate that has a box shape opening on on end and a slot cut on the other end that sits on the left side forward in the trans. opening that just sits between two roll pins that stick up to set it on that allow it to move around a little. When I took the cover off it came off with it. There are also two removable plastic or nylon blocks that fit in a slot in the front and back of the the shifter assemlby. There is a groove cut on one side of each block towards the end of the block. They also came off during removal. I put the trans. back together installed the flat metal peace as I thought it should go, however there seems to be several ways it could go, and put the blocks in with the groove either up or down I forget right now. Since then, I've had four occasions of the transmission not going into reverse. Did all the clutch pedal stuff to try and syncronize or move the gears with no result. After driving for a while it goes into reserse normally until it happens again. Something is getting in the way and then moving into an o.k. position. I don't want to do the trial and error thing until it stops if someone can help me. I removed the shifter plate and top trans. access plate and am ready to intall things properly if someone can show or tell me how these parts need to be if in fact that's my problem.

When you say "cover plate" do you mean the entire top cover of the transmission complete with the shift forks?

Here's a service manual for 1993, perhaps it's the M5OD transmission?

I don't see anything specific to your problem, but check through there, and if it's the same tranny, perhaps we can work on it with you

Or is it the S5-42 ZF ?
