They don't exist - from the factory.
Centurion conversions did 4-door crew-cab modifications on F150 platforms, so they are out there, but rather than drop a crew cab HD F series body on, they actually cut and spliced cabs together for it.
They made top rate stuff, including some Broncos. Run a search for Centurion and you'll get a few member rides.
