6.0 cutting out

i have a 2003 6.0 that has started cutting out around 2200 rpm's. It hesitate and blows black smoke out the tail pipe. Kinda acts like a gas burner with water in the gas. I have changed all fuel filters, and drained water separater. Also added fuel system cleane to tank. Is there any sensor that could be causing this.

ICP sensor? But if it is, I think usually the idle changes some too. Other than that, I would pull the EGR valve, and clean it really good...
