I have a 1996 Explorer. I have been having a Problem with Over Heating, When I opened the hood I noticed that My Hose from the Radiatore has Collapsed and looked like everything was sucked out of it... Both Bottom and Top Hoses,, when I took off the Cap eveything sprung back into place,,,
I gave Changed the hoses, Replaced the Thermostate. Flushes The Radiator Two times..
It could also be a plugged or crimped line between the rad and the reservoir. Sometimes the res can collect rust or crud in the bottom, and it gets sucked back into the line when everything cools. Lines also sometimes get pinched.
It's more likely what has already been suggested though. Whenever I have to hunt down a cooling issue the cap is the first thing to go by-by, I never try to reuse them - they go directly into the garbage can with no stops on the way. Since they're cheaper than a gallon of antifreeze, I don't even think twice. If I don't know when it was put on brand new it's history!
It's also a good opportunity to install one with a safety vent/pressure release if it didn't have one to begin with. Many stock radiator caps don't have a pressure release, and if you've ever been scalded you don't want it to happen any more!
