Do you have a loss in oil pressure once warmed up or a loss in power?
Now that I've heard the vid, I'd have to say that I think it sounds like a knock. What all have you used the truck for? Did you change the oil regularly? Did you run the **** out of it? What kind of oil have you been using (weight and brand)?
I dont think i have a loss in oil pressure, i might have a lilttle loss in power, but she still runs very strong, so i dont know. i have just used the truck for regular drving, i have changed the oil regulary, i havent really got on it a whole lot, every once and a while ill floor it. Umm, i have been using Penzoil, 30 weight.
Used to have a 300 six that spun a rod bearing. (then, I bought it!) Driving it home, I was able to silence the knock by backing off the throttle as much as possible and maintain speed. It ran quiet most of the trip, 40 miles, and with a reconditioned crank, bearings, and one rod and piston, it ran another 200k for me over 14 years.
Of course, I had a garage to work in at the time.
You might need to know how low your oil pressure is at this time.
I can't tell from the video. But here are some suggestions.
1. Take off the belts, you can run it a minute or two if you start off with a cold engine. That will get rid of the acceory noises.
2. Rod bearings will sometimes give you a double knock when you rev it up and let off.
3. Main bearings will give a deep rumble.
4. At 153,000 miles I'd suspect a cam chain that has stretched and is fluttering.
5. If it gets worst when the engine oil is up to temp "please don't try this with the belts off" then its a bearing.
6. If its worst cold then piston slap.
7. Things that go knock: Rod, main, wrist pins, lifters, cam chains, Pistons, loose flywheels, Loose bolts that hold things on and dont forget the exhaust bumping on something.

Thanks, it must be piston slap, because once it is warmed up it quiets down. How do you fix piston slap?, i don't really want to get a new engine for it yet...
