remove the throttle linkage to the TB, now manually open it - doe it return to close on its own? (I have a 90 TB with perfectly fine springs that binds in the throttle shaft due to gunk built up where the same thing happened)

Mine does something similar but it is a loss of electricity for an instant. I'll step on the clutch or engage the turn signal and blip ... off/on and there go the high revs as though I just started the rig.

Measure the TPS output through it's whole range, it's possible the sensor has a dead spot at part throttle. This would explain code 12 you are getting.
