I used to have one like this, was a 74 f-250 4x4 with a 79 supercab cab on it. I think they just moved the rear cab mounts back and used a body lift on the front of the cab and core support to make up for how the frame is bent. If I still had it I'd take some pics. It was cool, had a short stake bed.
