It was a guy at my work that manufacture the adaptor in a machine that we call "thread-spark" dont now the American name for it.
Yes... Thread Spark would mean here Wire EDM... EDM means Electic Discharge Machining...
This is a computer controlled machine where a thin ... most likely .010 inch dia ... brass wire is used for erosion of metal. a controlled electric current is applied through the wire and the wire is moved in a specific controlled manner to generate a shape. these machines are capable of accuracies of ±.0001 inch.

This is about the only method I could see for him to produce the spline pattern on both the OD and ID of the adaptor. other methods are machining with a mill on the OD, and broaching the ID ... but the tooling cost for the broach would be hiIII....
The pilot on the inputshaft is still used,changed the pilotbushing on the crank to a pilot bearing with smaller ID.
The throwout retainer did I fabricate in a metal lathe, cut the old one of and
made a new one.The new one is "shrink fit" on to the part that is attach to the transmission.
