Most states have a provision for making the mileage on the title "possibly inaccurate" due to cluster swaps, overall age, etc. It's not uncommon for a 10-15 y/o vehicle to have inaccurate mileage between the title and the odometer. Less so nowadays but this was address in law 20 or 30 years ago when such things were more common.

that truck is probably geared so that when it trips the lights at the end of the 1/4, the engine will be turning max rpm..it'll probably have shifted into high gear 4-5 seconds earlier....if they buit it right, there is not enough info in the ad. Chevy used 2spd trannys for a lot of their cars in the '60's & '70's and they worked just fine in a daily driver. You decide if you want to drive that on the hyway
