Wiring Linear actuator

Need a little help !
I have a linear actuator with no directions. The motor is labled 12v also says Blue & Geen wires to motor/ red,black,yellow are +5v ground sensors.
No other info avialible.
I have found that green pos./blue neg. = motor out
Blue pos. / green neg.= motor in
any help on how or what the groud sensors are for, or how to wire this thing up would be great.

I assume that the red/black/yellow are the sensors for all the way out and all the way in. surprised they take +5.. I would have expected ground.

Thanks Sam,
after some research I've found that the three ground sensor are for a switch sytem if you want the actuator to stop in various differnt positions other than full open or full closed.
If I want only full open and full closed then I think I need to wire though two relays to reverse polarity of the green and blue. Does That sound right ?

wired it all togather on the bench with two relays works great.
