Have a 04 5.4 #3 cyl. sparkplug broke put a hole in the piston. My brother inlaw works at a ford dealer said he has never heard of it before. anybody out thier knows what caused this.

I had this happen to me I have an 05 F-150 and one of the coils went bad so while at the dealer they were removing and replacing all the spark plugs and one broke falling into the piston. The truck was not running so no further damage was done but they ended up pulling the entire engine and removing the heads in order to get it out. The truck only had 50k miles on it and I didn't have the extended warrenty. After three weeks Ford decieded to take responsibality and swallowed the $4800 cost for all the work. I was nervious the truck would not be right again but it is as good as the day I bought it.
Sounds like the plug broke while the engine was running is about right.
So, let's see...in addition to losing sleep over breaking a spark plug while changing them while the engine is off and having to pull the heads, I now have to think about a plug breaking while the engine is RUNNING causing me to have to pull the heads/engine? Anyone have any Ambien handy? I'll be gobbling them like chicklets......
Honestly I don't think it's a common enough issue to really worry yourself about. You're probably more likely to throw a piston through the oil pan than to have that kind of random mechanical failure in the engine.
Keep in mind, there are millions of these trucks on the road...
