Can you tell if the problem is in the shifter linkage? Does the shift lever move at all when it jumps out of gear?

Check your oil level condition, you need to run the car to temp- drive it, park, pop the hood- leave it running in nuetral and pull the dipstick.........the oil should be pink and transparant if its not or is low, you will have to do a flush and change.
This is time consuming as to do it properly you will need to drain and fill it a few times.
This should be the first thing you check, then the linkage as mentioned, make sure the linkage is free to move- lubricated and not bent up or fouled in its movement range.
Let us know what you do and how you get on please.

I agree, when was the last time this tranny saw a fluid change, as in a complete fluid change.
