Reflash's are usually used by the manufacture to make changes to your PCM based on updates or issues they have found. Usually you will be notified by your dealer or manufacture if your vehicle needs this.
Some reflash's you wont know about untill your vehicle demonstrates a certain problem and you take it to the dealer. The dealer might have a TSB on your issue that requires a reflash for updates and so on.

So must have been an update. So that is 3 updates. The second one I had done twice I don't no how but I had a lose of power and bad shifts and they said it took it.

The fire issue was caused by faulty injectors damaged at the factory when they were exposed to reverse polarity during testing. This problem combined with the prior regen tuning caused the fire issue.
