Most window cranks are held on my a clip that slides into a notch in the shaft. If that's the case with yours, try wrapping a rag around the shaft behind the crank and pulling it around. The rag will usually catch on the clip and pull it out.

The crank handle is a two part piece. Get a flat screwdriver on the edge of the shaft part and it should lift off the cover, which you can rotate out of the way, but don't pull it or you'll break it. Then you'll need a long torx driver to get the bolt that holds it down. If you need to pull the door panel off, pull it UP, not out (after removing the other screws that hold it)
