Melted plugs again

02 F 250 SD 5.4 auto. at 68,000 miles #3 plug shoots out, repaired it with tim-sert, then had miss in engine 5,000 miles later, turns out 6 plugs and some valves was melted, got engine rebuilt with tim-serts installed in all plug holes, injectors were pluged with dirt, replaced all eight, had fuel system and tank cleaned. ran fine but had to use highgrade gas or it would ping.
now 7,300 miles later started missing shot codes P1299, PO174,po301,PO302,PO306,PO308,PO305. took out plugs all melted (bad). Out of 40 Super Dutys in the fleet this is my problem child. whats next? any ideas?

My initial guess would be that the engine is running way too lean, thus hot.

If you've had injectors clogged before, I'd be looking for improperly-filtered fuel. Maybe an aftermarket two-micron in-line filter is in your future?

It could also be a malfunctioning O2 sensor causing the lean condition.

Changed O2 sensors at rebuild, so It did it on old and new. I'm going to check fuel filter next and see if anythings in it, Then see if the cats plugged. Thanks for the input guys.

With only 68k on the truck the cat should not be plugged.

I agree it sounds like its running lean. I would check for any leaks in the intake or vaccum leaks. The MAF could possibly be bad because that is what monitors how much air is coming in so the proper fuel can be added.

Did you put in a reman engine or just get the heads rebuilt.

I'd looking into getting a wideband o2 installed before I really hurt the engine again.

dkf I Checked for vac. leaks already, swapped MAF with another truck, rebuilt the factory engine that was in the truck.
hott-rodd where do I get more info on a wideband o2? took this thing to dealer after all the work was done the first time just to get it checked and they said everything was fine. after changing plugs this time took it to another dealer they say its fine, I know its not that this will happen again. Thanks again guys

You might try checking the fuel pressure.
