A very very low gear in the transmission that is good for when the going gets tough. It's so low you can put it in granny low and let out on the clutch without touching the gas pedal, and the engine will not stall.
So if someone says they have a granny 4 speed, in useage they really have a 3 speed with a very low gear they only use rarely. It's so low, you never use it on the road, and 99% of the time you start out in 2nd gear for normal driving.

Bit of advice: Buy a set of Ford Shop manuals (especially Vols.A & B) or at least a Chilton's or Haynes manual and the original operator's manual for your truck. It'll save you lots of time and money too. Most are available on eBay Motors Auction. You have to keep checking back or make it a "Favorite" search.
When I was younger, I would throw my old 83 F-150 4x4 in granny 1st and throw the t-case in 4-low and walk downhill faster than the truck would idle down the hill. I used to love to freak out girls who went 4 wheeling with me by doing that.
I did that until I watched my buddy do the same thing with his girlfriend in it, only his tranny popped out of gear and he chased that thing down the hill cussing and stumbling the whole way while Kim screamed like an air raid siren all the way down the hill. I laughed so hard I almost pissed myself. Needless to say, those two weren't together too long after that.
